Notes:
  1. Username is prefilled with first 15 characters of email prefix from RockYou
  2. If no mobile information is detected from the user's RockYou profile then the mobile info form is added to the page. If the phone info is detected then the mobile info form is still added to the page however the fields are filled in so the user may change them if necessary.
  3. On submit:
    1. the username is checked against existing users in the FM userDB
      1. If the form has any errors then errors will be shown inline with the form with "fix" messaging
      2. If the username is uniques the credentails get stored in a temporary FM userDB with a RockYou flag indicating the subcription originated from RockYou
        1. If the username is not used within 15 days of creation it is scrubbed from the temp DB
      3. The selected image references from RockYou are stored in the temp DB
      4. The user is sent an email (on file at rockyou) with download instructions based on carrier to get aPix on their phone and complete the subscription
      5. User is redirected to a web page with download instructions
